Le devenir des accidents de la route est un probl\u00e8me de soci\u00e9t\u00e9 dont les chiffres de mortalit\u00e9 ne suffisent pas \u00e0 rendre compte\u00a0; si l\u2019on \u00e9voque souvent les d\u00e9c\u00e8s caus\u00e9s par les accidents, on parle moins souvent des handicaps qui affectent de nombreuses victimes survivantes, et pratiquement jamais des handicaps \u00ab\u00a0invisibles\u00a0\u00bb. Les s\u00e9quelles peuvent avoir une importance suffisante pour retentir sur la victime et sa famille\u00a0constituant ainsi le groupe des bless\u00e9s graves, alors m\u00eame que l\u2019on ne sait pas comment le d\u00e9finir. C\u2019est sur cette question que porte particuli\u00e8rement notre recherche.<\/p>\n<p>La cohorte prospective ESPARR, fond\u00e9e sur le registre des victimes d\u2019accident de la circulation du Rh\u00f4ne, est l\u2019une des rares au monde \u00e0 traiter du sujet des cons\u00e9quences de l\u2019accident de la route \u00e0 moyen et long terme. Ses objectifs sont d\u2019identifier les facteurs pronostiques du handicap, et des probl\u00e8mes de r\u00e9insertion des victimes d\u2019accidents de la circulation. Du fait de son caract\u00e8re unique, la cohorte ESPARR et les recherches th\u00e9matiques qui pourront en d\u00e9couler, constituent un soutien scientifique fondamental pour d\u00e9finir des mesures concr\u00e8tes et adapt\u00e9es.<\/p>\n<p style=\"text-align: center;\"><strong>ESPARR ou pourquoi s\u2019int\u00e9resser aux cons\u00e9quences d\u2019un traumatisme caus\u00e9 par un accident de la route ?<\/strong><\/p>\n<p><a href=\"http:\/\/www.revarrhone.org\/wp-content\/uploads\/2017\/06\/Esparr.png\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter  wp-image-1145\" src=\"http:\/\/www.revarrhone.org\/wp-content\/uploads\/2017\/06\/Esparr.png\" alt=\"ESPARR ou pourquoi s\u2019int\u00e9resser aux cons\u00e9quences d\u2019un traumatisme caus\u00e9 par un accident de la route ?\" width=\"529\" height=\"386\" \/><\/a><\/p>\n<p>L\u2019objectif de la recherche est d\u2019\u00e9tudier l\u2019\u00e9volution sur 5 ans et le devenir des victimes d\u2019un traumatisme cr\u00e2nien mod\u00e9r\u00e9 ou s\u00e9v\u00e8re en terme d\u2019impact personnel (s\u00e9quelles et handicap physique, retentissement psychique, affectif, professionnel) familial (impact socio\u00e9conomique sur la famille et rupture ou renforcement des liens), et soci\u00e9tal (\u00e9valuation de l\u2019impact pour la collectivit\u00e9).<\/p>\n<p>Un objectif important d\u2019ESPARR est de r\u00e9pondre \u00e0 la question r\u00e9currente sur le bless\u00e9 grave \u00ab\u00a0Qui est-il\u00a0? Comment peut-on le d\u00e9finir\u00a0?\u00a0\u00bb. Nos r\u00e9sultats devraient nous permettre de lancer cette r\u00e9flexion bas\u00e9e sur la synth\u00e8se des divers indicateurs que nous avons mis en place.<\/p>\n<p>L\u2019inclusion des sujets dans la cohorte s\u2019est d\u00e9roul\u00e9e sur une p\u00e9riode d\u2019un an entre octobre 2004 et d\u00e9cembre 2005 dans les h\u00f4pitaux publics et priv\u00e9s du d\u00e9partement du Rh\u00f4ne, lors des premiers soins apr\u00e8s l\u2019accident. La cohorte a fait l\u2019objet d\u2019un suivi \u00e0 court terme (6 mois apr\u00e8s l\u2019accident puis 1 an), \u00e0 moyen terme (2 et 3 ans), et \u00e0 long terme (5 ans), avec \u00e0 chaque \u00e9tape une investigation par auto-questionnaire adress\u00e9 \u00e0 tous les sujets.<\/p>\n<p>Cette information de base a \u00e9t\u00e9 compl\u00e9t\u00e9e pour des sous-groupes particuliers par des informations acquises lors d\u2019une rencontre en t\u00eate \u00e0 t\u00eate (une investigation clinique comprenant une \u00e9valuation fonctionnelle et cognitive) : &#8211; patients traumatis\u00e9s graves (ayant au moins une l\u00e9sion AIS3+)\u00a0; &#8211; patients traumatis\u00e9s cr\u00e2niens (pr\u00e9sentant au moins une l\u00e9sion \u00e0 la t\u00eate: AIS2 et plus).<\/p>\n<p>\u00a0ESPARR \u00e0 un <a href=\"http:\/\/esparr.inrets.fr\/\">site<\/a> d\u00e9di\u00e9.<\/p>\n<hr \/>\n<p><span style=\"color: #0000ff;\">P<em>\u00e9lissier C, Fort E, Fontana L, Hours M.
